I authorise WTC staff to contact emergency services in the event of an emergency; obtain other medical assistance deemed necessary in the event of an emergency/accident, including the administration of an anaesthetic or the carrying out of necessary surgical procedures by a qualified medical practitioner.
I agree to pay all medical and dental expenses incurred on behalf of the above participant(s).

I agree that if my child misbehaves, becomes ill or for any other reason deemed appropriate by workers, I will collect them from the activity when requested by staff to do so.
I consent to my child/ren having their photo taken whilst participating in West Tamar Council (WTC) Youth activities for the purpose of being used for YAC promotions, media, and other council publications.
